WHEREAS Hospice Palliative Care Week 2011 is focused on raising awareness that the responsibility of caring for someone with a life-limiting illness is one that needs to be shared collectively within your community. Together, we can all make a difference in the lives of individuals who are living with a life-limiting illness and those affected by it.

Quality hospice palliative care helps patients and their families address medical practices issues and their physical, psychological, social and spiritual needs. Staff address the expectations, needs, hopes and fears of their patients and provide counsel on self-determined life closure and the dying process, and assist family members with their feelings of loss and grief during illness and bereavement.

The hospice palliative care networks within the City of Toronto and their friends and partners seek to ensure that hospice palliative care for its patients, caregivers and their families is integrated and of high quality. Continued commitment, awareness and partnerships are key to this vision.

NOW THEREFORE, I, Mayor Rob Ford, on behalf of Toronto City Council, do hereby proclaim May 1 -7,2011 as "Hospice Palliative Care Week."
