Philip Aziz Centre (PAC) is a non-profit hospice that provides practical, physical, emotional and spiritual support for adults and children living with life-limiting illnesses and their loved ones, in the comfort of their homes.

PAC provides interim, respite and palliative care, allowing people to live their lives fully to the end with dignity, respect and comfort, surrounded by a loving circle of support. PAC services support individuals and their loved ones throughout the trajectory of illness from diagnosis to bereavement and are delivered in collaboration with health care professionals and other supporting agencies involved.

PAC is involved in community education and strongly endorses a health care system that is patient-centered, community based and responsive to people’s healthcare needs in an excellent, timely, accessible and accountable manner.

To ensure good standards of practice PAC has achieved Level 2 Accreditation, (currently the highest level offered by the Hospice Association of Ontario).

PAC staff and volunteers are skilled and compassionate in the provision of hospice care for adults and children and are committed to respecting confidentiality, beliefs, choices, faith and culture.
