Home-based Hospice Care in Toronto and the GTA

Philip Aziz Centre (PAC) is a non-profit, home hospice program providing practical, physical, emotional and spiritual support for people living with cancer, HIV/AIDS and other life-limiting illnesses.  Hospice services are delivered through a small professional staff team and large base of trained volunteers, who work in collaboration with healthcare professionals and other agencies involved in client’s care.

Hospice Care

Hospice Programs include the following:

  • Palliative care
  • Interim support
  • Caregiver relief
  • Respite
  • Children’s volunteer support programs
  • Professional chaplaincy service
    (spiritual and bereavement support)
  • Monthly spiritual care groups for HIV positive men, women and children
  • Recreational and camp programs for children

All hospice services are provided free of charge.

PAC is a member of the Hospice Association of Ontario/Canadian Hospice and Palliative Care Association, Canadian Network of Pediatric Hospices, The Toronto Central Palliative Care Network, Ontario Multi-faith Council, and an Associate member of the Ontario AIDS Network.

PAC’s programs are generously supported by the Ministry of Health, numerous foundations and organizations, businesses, service clubs, churches and individuals, all who help to meet current needs and allow for expansion as the demand for hospice services continues to increase.

Emily's House: Our Future Children's Residential Hospice

children PAC is in the process of expanding its current hospice programs to children and families by opening Emily's House, a Children’s Residential Hospice. Emily's House – the first hospice of its kind in Toronto – builds on PAC’s fifteen years of care and support for people living with life-limiting illnesses. Emily's House will provide pediatric palliative care for 10 children living with a life-limiting diagnosis.
