Home-based Hospice Care in Toronto and the GTAPhilip Aziz Centre (PAC) is a non-profit, home hospice program providing practical, physical, emotional and spiritual support for people living with cancer, HIV/AIDS and other life-limiting illnesses. Hospice services are delivered through a small professional staff team and large base of trained volunteers, who work in collaboration with healthcare professionals and other agencies involved in client’s care.
Hospice Programs include the following:
All hospice services are provided free of charge. PAC is a member of Hospice Palliative Care Ontario/Canadian Hospice and Palliative Care Association, Canadian Pediatric Residential Hospice Network, Toronto Central LHIN Palliative Care Network, Ontario Multi-Faith Council, and associate of Ontario AIDS Society. PAC’s programs are generously supported by the Ministry of Health, numerous foundations and organizations, businesses, service clubs, churches and individuals, all who help to meet current needs and allow for expansion as the demand for hospice services continues to increase.
